If you've ever been to our Back2School Block Party before, you know in the past we have had so many great activities and screenings for the whole family.

While we are disappointed that we can't all get together in person, we can still offer you some great information and resources through our Virtual Health Fair!

By participating, anyone 18 years of age or older, can enter into our drawing to win a $100 Wal-mart Gift Card! Just click on a link below to enter.

The information you provide is confidential and will be used to help us plan our community heart health education screening programs. Thank you for participating.
