In order to better serve the parents and guardians at our Back2School Block Party, we would like to learn a little more about you. The information you provide is confidential and will be used to help us plan our community heart health education screening programs. Thank you for participating. NOTE: Registration for this event automatically enrolls you for our free email newsletter. Opt-out instructions are available in each email.
